Home Health and Hospice

Polson, in conjunction with St. Joseph Medical Center. Home Health professionals offer full-range of in-home medical services. Medicare, Medicaid, private pay and private insurance are accepted. Hospice Services begin when patient and physician decide against further treatment or when no curative treatment is available. Hospice care is provided at home, in nursing homes, assisted living facilities, or even hospitals for limited periods. Hospice is a benefit of Medicare.

Two types of home health: skilled and non-skilled.

Skilled: often after discharge from acute care; usually physician orders and care is short-term.

Non-skilled: in-home assists with daily living allowing patient to stay in their homes as long as
possible.
